If you wish to authorize the disclosure of your personal information to another healthcare provider for the purpose of coordinating your care, this may occur only with your informed written consent.

In accordance with the requirements of the Ordre des psychologues du Québec (OPQ), client records are retained for a minimum of five years following the last professional service provided.
